Allegation of Complaint: Dr. Muhammad Shakeel AwaisiNature of Complaint: Unknown at this time.Action Taken: The Physician has entered into a Voluntary Agreement Not to Practice Medicine with the board until further order by the board.September 17, 2008 The physician violated patient confidentiality by accessing confidential patient information through the hospital’s computer information service using the patients information for personal purposes. The emergency room patients with orthopedic injuries and complaints were told that they were referred by the hospital emergency department to the physician’s company for physical therapy. The physician also failed to provide requested information to the board.

The board has terminated the physician’s voluntary agreement not to practice medicine.

The board has Revoked the physician’s license to practice medicine in the State of Massachusetts retroactive to the date on which the board had accepted the physician’s voluntary agreement not to practice medicine.
